What Caused the Deadly Metro Subway Crash in Washington, D.C.?

The deadliest Metro subway crash in Washington, D.C. history occurred on Monday, June 22, 2009. A Washington Metropolitan Area Transit Authority (WMATA) Metro train full of commuters heading home during the afternoon rush on the Red Line collided with another train that was sitting idle killing nine and injuring 76 people.
The NTSB has discovered evidence that the train was in automatic mode and should have stopped with the system’s built-in safety mechanisms that would have prevented this type of collision from occurring. There is further evidence that the train operator, Jeanice McMillan, had activated the emergency brake prior to the collision; however, the train failed to stop in time. In today’s press conference, the NTSB did not identify any maintenance discrepancies and is still unclear as to why the collision occurred. However, speculation is still focused on the outdated train involved in the collision and sensors along the track that act as a fail-safe to prevent train collisions.
On Good Morning America, Washington, D.C., Mayor Adrian Fenty voiced his opinion and is placing blame on local officials for their failure to ensure the safety of D.C.’s mass transit system which has been Washington’s crown jewel for transporting commuters and tourists throughout the city.
